UCSF policy requires specific forms be attached to MyExpense expense reimbursements based on the travel destination and vaccination status. 

Travel to another state: 

• If up to date on COVID vaccines: no approval needed. For reimbursement, fill out the Vaccine Attestation Form. 
• If NOT up to date on COVID vaccines: The Pandemic Travel Approval Request Form with appropriate signature is required by central campus leadership for travel approval and reimbursement.  

Travel to another country (regardless of vaccination status):

• The Pandemic Travel Approval Request Form with appropriate signature is required by central campus leadership for travel approval and reimbursement.  

If the required forms are not attached, the expense reimbursement may be returned to the traveler. Be sure to attach any necessary forms to your expense reimbursement to facilitate payment.
