The Substitute W-9 & Supplier Information Form has been updated to include details for payment method and payment terms for individuals. The updated form can be found on the Supply Chain Management website. Please discontinue using previous versions of the Substitute W-9 form.

As a reminder, the IRS Form W-9 is not sufficient for supplier onboarding and cannot be accepted in lieu of the UCSF Substitute W-9 & Supplier Information Form.
