Request for Service/Warranty Repair

If you have furniture that needs repair, follow these steps to see if it is covered under a warranty and/or how much repairs would cost:

  • Fill out the Request for Service/Warranty Repair form with all the pertinent information, most importantly the model number and ship date on the tag – please send photo of the tag when emailing the service form if there is one. Example: A Steelcase chair has a tag on the bottom of the chair that should has this information. 
  • Email the form to Matt Cooley at OWP, who will enter an open issue with the vendor so they can advise if this item is under warranty or not.
  • If the vendor is Steelcase and the item is under warranty, then Steelcase will pay for the labor and parts. Some other vendors only pay for parts and not labor, but OWP will advise you if that is the case.
  • OWP will let you know if it is still under warranty and let you know that the parts are being ordered and service will be in contact with them to arrange a date to come out and repair.
  • If the product is not under warranty, OWP can quote the parts and labor to UCSF at a cost so they can determine if they want to proceed. Anything not under warranty is billable at $144 if a service call is made and the product not under warranty. The charge is noted at the bottom of the service form
